NICE guideline on delivering intermediate care

Image: NICE logoNICE has produced a guideline to support the delivery of intermediate care. The guideline covers the core principles of intermediate care, assessment of need, referral, entering the service, how to deliver care, transition from care, and training and development for health and social care practitioners.    

Intermediate care provides multidisciplinary support and rehabilitation to people at risk of hospital admission or who have been in hospital. It is designed to facilitate timely transfer from hospital to community, and prevent unnecessary admissions to hospitals and residential care.
