Supporting older people with multiple long-term conditions
The Social Care Institute for Excellence (SCIE) is working with NICE to identify and gather a number of practice examples to support the implementation of the NICE guidance for older people with multiple long-term conditions.
They are looking for examples that demonstrate high quality practice specifically relating to older people with multiple long-term conditions (or that can easily be translated to the needs of this group of people).
The areas they would like examples for are:
- Named care co-ordinator role.
- Self-management of long-term conditions and preventing social isolation.
- Integration of different care and support options to enable person-centred care.
Some of these examples will also appear in the Shared Learning section of the NICE website.
