Examples of emerging and developing practice when using a high impact change model

LGAA new resource provides examples of emerging and developing practice to support the Local Government Association (LGA) and Association of Directors of Adult Social Services (ADASS) high impact change model for managing transfers of care.

The resource - high impact change model managing transfers of care - examples of emerging and developing practice - brings together examples of work being undertaken across the country for system change. In particular it references a range of initiatives where there is already evidence of impact, points to examples of emerging practice, and includes links to published guidance and further information.

This new resource supplements the earlier high impact change model, which identifies eight system changes that will have the greatest impact on reducing delayed discharge.

In order to continue developing the evidence base for this change model, LGA and ADASS are regularly reviewing and updating the resource and are also welcoming contributions from practice. To provide examples contact chip@local.gov.uk.
