About the study
The Geriatric Resources for Assessment and Care of Elders (GRACE) model of primary care was evaluated in this American randomised controlled trial. The model aimed to improve integration of geriatric and primary care services for adults aged 65 years and over on a low income. It provided two years of home-based care management by a nurse practitioner and a social worker in collaboration with a primary care physician and geriatrics interdisciplinary team. The impact of the intervention on service users’ health outcomes was assessed.

Findings
Patients who received the intervention improved significantly compared with the control group in four of eight SF-36 scales: general health, vitality, social functioning and mental health. Hospital admission rates and numbers of visits to Accident and Emergency departments were significantly lower among these patients in the second year after receiving the intervention.
