About the study
This study discusses the importance of providing a wide spectrum of joined up, accessible services for all groups of older people. It explores a whole system model that puts the individual older person at the centre of service planning and delivery using case studies and giving tools and advice for developing such a service.

Findings
The study discusses the importance of developing a 'whole systems persepective' by developing a shared vision, mapping services and the experiences of older people, and redesigning services to streamline access and reduce duplication. It also highlights the need to ensure that there is a clear pathway so that people can be guided through the system easily.
